I know you can report the post to Tumblr, but it can take them a while to respond. Do you know any of their Tumblr, meatspace, or Facebook friends? If so, what I have had to do in the past is contact them and ask them to try and get in touch with this person. If you don't know any of them, you can sometimes search for posts in which they're tagged, and if they seem to be tagged a lot by a particular person, that might be someone they're close to who has their number/knows someone who does/knows where they are and can call for help.
You might want to log off Tumblr for a little bit and get some rest and water. I've sadly been in this situation a lot before and know how stressful it can be. Sometimes it's good to take some time to yourself afterward.
