It's Lucky Felicia, the door-opener! Last time we saw her she was in fact opening a door, which is coincidentally when she lost her arm.
Coincidentally.

 

To be fair, the last time we saw her she was wearing a costume that didn't consist mostly of blood.

I thought that design was too quirky and thoroughly considered for her to be killed off so quickly. That was too distinct a character.
