Well, that's good because the first thing I was going to say is that 前 / 後 is used to indicate a.m./p.m. on forms usually so it didn't make much sense in that context.

You are assuming a knowledge base that the assessment takers will not have. Even just indicating pre-assessment and post-assessment does not convey any real meaning. You'd need to explain that there are going to be two different forms. Why does that particular line need to be on the form at all if it's not meant for the assessment takers to interact with? I mean, usually that sort of internal information is put at the end of the form with a "For [office] use only; do not write below this line." And wouldn't need translated.
Mostly, I wanted to include that line because a. I want to make sure the same information is on the english survey form and any other language survey form. It's a little thing, but I want to be sure everyone has the same information even if they're uncomfortable with the language. b. it also needs to be on the form so that we can enter the information into our systems. Both of these reasons can be worked around if I remove pre/post, so if it's an issue re: translation, it absolutely isn't a part of the survey as much as it's an ID marker for this batch of surveys, and we can remove it.
